Palliative Care Nurse

Summary

The Upper Grand Family Health Team (UGFHT) is seeking a Palliative Care Nurse to provide support for patients and their families requiring palliative care. This is a maternity leave contract position, 0.9 FTE, based in Fergus, Ontario. The successful candidate will work within an inter-professional team, collaborating with a Palliative Social Worker and community partners to deliver high-quality end-of-life care. Responsibilities include holistic assessment, symptom and medication management, caregiver support, advance care planning, and interdisciplinary communication. Occasional after-hours support is required, and a valid Ontario Driver's license with access to a vehicle is necessary.
