Pharmacist
Summary
The Carefirst Family Health Team (FHT) is seeking a full-time or part-time permanent Pharmacist to join their interdisciplinary team. This role involves providing education on pharmacological therapies, offering consultative advice on medication management, and supporting medication reconciliation. The Pharmacist will manage complex medication regimens, identify drug interactions, and act as a drug-related information resource. Responsibilities also include collaborating on care plans, contributing to clinical program development, and driving quality improvement initiatives. The position requires current Pharmacist licensure (Part A) with the Ontario College of Pharmacists and at least two years of recent clinical experience, preferably with geriatric experience. Additional certifications or experience in dementia care, health promotion programs, and cultural sensitivity are assets. Proficiency in Microsoft Office and strong communication, analytical, and client service skills are necessary.
