Nurse Practitioner (Primary Care)

Summary

Langs is seeking experienced Nurse Practitioners to provide primary care services within Cambridge Family Health Organizations (FHOs), including Cambridge Coronation FHO, Forbes Park FHO, and Delta Coronation FHO. While employed by Langs, the Nurse Practitioners will work day-to-day within the assigned FHO, following its specific policies and schedules. The role involves delivering comprehensive primary care, focusing on serving vulnerable populations to improve access and reduce health service barriers. Responsibilities include assessment, diagnosis, treatment, health education, documentation, and contributing to program development and evaluation, all while adhering to the College of Nurses of Ontario standards. Experience with complex patients, chronic disease management, neonatal and pediatric care, and multidisciplinary teams is highly valued.
