Nurse Practitioner (Primary Care Connect Clinic)
Summary
The Cambridge North Dumfries Ontario Health Team (CND OHT), in collaboration with Langs, is seeking a Nurse Practitioner for its Primary Care Connect Clinic. This part-time, casual contract position involves providing short-term support to patients who are not currently attached to a primary care provider. The core mandate of the role is to prepare these patients for a successful transition to a permanent primary care provider by stabilizing urgent conditions, initiating care plans, and addressing their health needs. The Nurse Practitioner will work to their full scope of practice, which includes assessing, diagnosing, and managing patient health needs. Responsibilities involve ordering and interpreting diagnostic tests, prescribing medications, developing and implementing care plans with an interprofessional team, and providing health education and self-management support. The role is crucial in ensuring patients are ready to transition into ongoing primary care by completing necessary investigations and referrals. Candidates must have a Baccalaureate in Nursing, a Primary Health Care Nurse Practitioner Certificate, and an unrestricted RN (EC) designation from the College of Nurses of Ontario. A minimum of two years' experience as a Nurse Practitioner in a primary care setting is required. Additional qualifications include experience with chronic disease management, community development, and working with diverse populations as part of a multidisciplinary team.
